Services Provided

  • Issuing of Birth/Marriage/Death certificates. (Possible to issue Birth Certificates registered in all the districts between 1960 to 2016 and all the certificates registered in the Divisional Secretary’s Division-Panadura)

Registration of a past birth

  • Rs.60/- is charged for the registration of a past birth.
  • If a birth has not been registered within three months since it’s occurrence;
  • It could be registered by submitting the following documents to the Additional District Registrar before the child completes the first year.
  • After one year of an occurrence of a birth, approval of the Land Registrar- kalutara is required for the registration of that particular birth.
  • Submit only the originals (Certificates obtained from the Divisional Secretariat) of the following documents.
  1. Birth Certificate of father
  2. Birth Certificate of mother
  3. Marriage certificate of the parents
  4. Birth Certificates of other children
  5. Nil result sheet
  6. Letter citing reasons for the delay in writing the birth
  7. An Affidavit if there is any difference in the names
  8. Copy of the Health Development Record
  9. A date will be set for the assessment. Father, mother and the child has come to the office on the given date.

 

Alterations of the Father’s section/Mother’s section in a Birth Certificate

  •   Submit only the originals (Certificates obtained from the Divisional Secretariat) of the following documents.

  1. Birth certificate of the person relevant to the alteration, obtained from the record room/Archives.
  2. Birth Certificate of father
  3. Birth Certificate of mother
  4. Marriage certificate of the parents
  5. Birth Certificates of other children
  6. Originals of National Identity Cards along with photocopies (of father and mother)
  7. Declarant’s (Person who submits the documents) Certificate of residence
  8. An Affidavit if there is any difference in the names
  9. Might have to submit other documents depending on the requirement of the alteration.
  10. A file

Change of Name in the 2nd Row Below Seven years of age

  •   Instructions to fill the form

  1. Do not use correction fluid.
  2. Place declarant’s signature in place of applicant’s signature. (There are three places to be signed)
  3. Write the name in full with the surname as it should be altered.
  4. Fill the form clearly and correctly.
  5. Whenever altering a wrong entry strike through with a single line and place applicant’s short signature in front of it.
  6. Get the form properly signed by a Justice of Peace and get him to place his official frank.
  •   Submit only the originals of the following documents.

  1. Copy of the relevant Birth Certificate with purple coloured official frank obtained from the office.
  2. Letter from Father/Mother (Not the applicant) citing their willingness for the relevant alteration.
  3. An Affidavit from father/mother stating that a single person is referred to by both the names. (Should place their signature on a Rs.50/- stamp and get it certified by a Justice of peace)
  4. Certificate of residence issued by the Grama Niladhari to confirm the declarant’s residence. (If your residence is not within the Divisional Secretary’s Division - Panadura, this certificate should be counter signed by the relevant Divisional Secretariat)
  5. Request Letter citing the necessity of alteration of name.
  •   Submit the Originals and a photo copy of the following documents

  1. National Identity Cards of parents.
  2. Parent’s marriage certificate.
  3. Birth Certificates of parents.

Change of Name in the 2nd Row Above 21 years of age

  •   Instructions to fill the form

  1. Do not use correction fluid.
  2. Always write the name in full with the surname.
  3. Fill the form clearly and correctly.
  4. Whenever altering a wrong entry strike through with a single line and place applicant’s short signature in front of it.
  5. The person applying for the certificate should always sign as the applicant.
  6. Signature of the applicant on the declaration form (B.10) related to the alteration of name and the affidavit should be certified by a Justice of Peace. Check whether his official frank is placed properly. (If the applicant is residing outside Kalutara District, the certification should be done by an all Island Justice of Peace.)
  7. The proof provided has to be tallied correctly with the requested name.
  •   Submit only the originals of the following documents.

  1. Copy of the relevant Birth Certificate with purple coloured official frank obtained from the Divisional Secretariat.
  2. An Affidavit stating that a single person is referred to by both the names. (Should place the signature on a Rs.50/- stamp)
  3. Submit the full page of the newspaper containing advertisement published by the applicant. (An alteration to the Surname or the Name could be done only after one year since publishing the newspaper advertisement)
  4. Certificate of residence of the applicant and recommendation of the Grama Niladhari on alteration of name. (has to be counter signed by the relevant Divisional Secretariat)
  5. Submit the applicant’s National Identity Card along with a photo copy. (If the N.I.C. is not available, has to submit a recent photograph certified on it’s back by the Grama Niladhari, as verification of identity.)
  6. Request Letter citing the necessity of Name change.
  7. In the event when the applicant is employed, a letter stating that the work place has been informed regarding the alteration of name.
  8. If the applicant is married, marriage certificate of the applicant.
  •   Submit the Originals and a photo copy of the following documents as proof of the name to be altered.

  1. Birth certificates of children.
  2. Copy of the Electoral Register.
  3. Driving License.
  4. Passport.
  5. Applicant’s bank pass book. (Has to be photocopied in a manner showing the name of the bank, Name of the individual, date on which the account commenced, date of the last transaction)
  6. Appointment letters
  7. School leaving certificate
  8. Certified copy of extract of the school admission register
  9. Applicant’s Deeds, title reports
  10. Letters of retirement
  •   Submit originals along with the photocopies of at least 5 of the above documents.
